Community Care Services

Community care services are an alternative to an aged care home and may be able to help you stay in the community.

If you need help with tasks such as gardening, cooking and cleaning, or bathing and dressing there are a range of community care services which may be able to help. You will need to check whether you are eligible, and what services are available in your area.

Commonwealth Carelink Centres

Commonwealth Carelink Centres are a new national initiative to assist anybody who needs information and contact details about community care services and aged care homes in their local area.

The Commonwealth Government has established Commonwealth Carelink Centres in 54 regions around Australia. Each Centre provides local information about services in your community - either over the telephone or face-to-face through a nationwide network of shopfronts.

Aged Care Assessment Teams (ACATs)

Aged Care Assessment Teams (ACATs) can discuss with you which type of care you need. They can refer you to community care services or help you access a Community Aged Care Package (CACP), aged care home or respite care.

Home and Community Care (HACC)

Sometimes you need help to continue to live at home. This might include some gardening, cleaning, or home modifications - installing ramps, or putting in a handrail in the bathroom. Or you may need regular help with everyday tasks like bathing, getting dressed, or preparing meals.

The Home and Community Care (HACC) Program provides a range of services to help frail older people and people with a disability to live at home.

Community Aged Care Packages

If you need a range of services like help with nursing, meals, shopping, and getting around, there's a specially designed service which can help you.

Community Aged Care Packages (CACPs) are individually tailored packages of care services which help people with lots of different care needs to stay living at home by co-ordinating the different services they need.

Respite Care

Many people have a family member or friend who helps them continue to live at home. These carers sometimes need support to keep going in their role. Your carer may need a break for a few hours or a few days.

Or you may need some extra help for a short period that your carer can't provide for example after an illness, to get back on your feet. Respite care is a chance for you and your carer to take a short break.

Day Therapy Centres

Day Therapy Centres are special centres, which offer physiotherapy, occupational, and speech therapy, podiatry and other therapy services to older people.
